I hereby consent that Florida Marching Band Championships (FMBC) may use or permit the use of any and all images of all participants in our group or property of our group shown in such images, in any and all media formats, and may copyright said images for sales, advertising, publicity or other business purposes all in such form and manner as FMBC may elect either in conjunction with our name or apart from it.*

I understand Florida Marching Band Championships (FMBC), all event venues and their respective employees, agents, staff, and representatives, will not be held responsible for any injuries sustained by any performing members, staff, or support personnel of the above-named group during or in conjunction with any performances or attendance at any performances of the 2026 FMBC competition calendar.*

This group and its sponsoring organization hereby agree to indemnify, defend and hold harmless (FMBC), all event venues and their respective employees, agents, staff, and representatives from any and all claims which might be brought by any person associated in any way with the above-described group, including performing members, in any way arising out of or during any performance of the group at any 2026 FMBC Event or in any way arising out of or connected with the attendance of said group.*

I understand that registration for State Championships is $800 if registered BY July 1st; AFTER July 1st, Registration Fee is $850.00 I will receive an invoice for payment upon completion of this form.*

I hereby certify that I have the authority to confirm compliance with the above items on behalf of the school, the band director and/or organization authorizing participation in this FMBC event.*

I agree to the performance draw dates and rules as outlined above.*
